Two things. First off, there’s the genetic problem, since with inbreeding there’s a higher chance of any children getting two copies of a lethal recessive. But an unrelated couple who are *known* to both be carriers of a lethal recessive would probably be at higher risk than a couple of overly-close siblings who have a clean genetic bill of health. But I would be okay with the unrelated couple having consensual sex, getting married. They would be highly advised to get some genetic counseling before having kids, but I wouldn’t force them into such counseling at gunpoint, or insist on some kind of forced eugenics.
Which means a bigger problem isn’t the inbreeding but the incest. I don’t have exact figures on this, and it would be difficult to get them in an objective fashion, but I’m willing to bet that the vast majority of cases of incest are not consensual, but cases of rape and/or sexual abuse. There’s just too many power dynamics going on within a family that we could say with great enough certainty that a given incestuous sex act is consensual.
